Most people will have an operation in their lifetime, but few of us have a clue what really happens once we are put to sleep. This series goes beyond the theatre doors of the surgical unit at the Queen Elizabeth Hospital Birmingham, where 700 operations are carried out every week, across 42 specialist theatres. Whilst many operations are vital but routine, this series focuses on some of the country's top surgeons as they perform high-stakes surgery that simply wouldn't have been attempted a few years ago, pushing the boundaries of modern medicine to its very limits. This episode features a painted wolf named Tait, matriarch of one of the last great families of this endangered animal. She has ruled her dynasty for many years on the banks of Zambezi river in Zimbabwe, keeping the peace within her sprawling family. But tensions are rising as Tait is growing old and her power is waning. One of her daughters, Blacktip, lives close by. Her own pack is growing fast and they are running out of space. She urgently needs to get more territory so that her pack can catch enough food. But she has a problem, she is hemmed in on one side by human lands and there is only one way she can go to get what she needs - her own mother's lands.

While sorting her dead grandmother's affairs, a young woman boards in an eerie, southern mansion and soon discovers the matriarch may desire more than just her company.

The young but travelled Ana arrives in a manor in the countryside of Spain to work as nanny of three girls and finds a dysfunctional family: the matriarch is a sick old woman obsessed by death and having constant nervous breakdowns; her son José was raised dressing in girl's clothes until his First Communion and is obsessed by military clothes and paraphernalia; Juan, the father of the three girls, a pervert since his childhood, writes pornographic letters to Ana; his wife, Luchy, has suicidal tendencies; and the mystic and religious eremite Fernando, who was inflicted to flagellation in his childhood, lives as a recluse in a cave. The presence of Ana disturbs the three brothers with tragic consequences.
